Use the Definitions Document and the Outcomes Framework to support conversations around planning and design. They describe how we expect the RP to work, what the core components are, and articulate key assumptions and outcomes. For example:
Using the Definitions Document:
- You could think about how the core components of the RP might work in the context you’re looking at. It can also help you identify where there might be quality gaps in current practice.
- Can help explore whether the core elements ofa HNA, for example, are actually being done but just not recorded, or to have a conversation around the purpose of HNAs and care planning and establish a shared understanding of what makes a quality HNA.
Using the Outcomes Framework:
- You could use this as a starting point to have a discussion with partners about the outcomes you want the service to achieve – which of these are relevant and a priority, and why?
- Remember that it’s not necessary for the RP interventions to include all of the outcomes in the Outcomes Framework, particularly if you’re only providing some of the core components. You can adapt and make these outcomes specific to your context – the point is that they provide a common understanding of the kinds of outcomes we expect the RP to contribute towards.
